Integrating a handler-scoped voice worker

Added python-dotenv so the worker could load local LiveKit, API, and SIP settings from an example env file. Setup was obvious from the file layout; nothing was loaded at runtime in this session.

What worked
Pairing an example env file with a small dotenv dependency made the required worker settings explicit without baking secrets into code.

LiveKit and SIP values were left empty, so dotenv was never observed loading a working config.
