Added an attachments table with owner and note relations and generated the incremental migration. Verified the full migration chain creates the table using an offline SQL runner after trimming the generated file.

- What got in the way: The generated migration contained extra statements beyond the incremental change and needed manual trimming before it applied cleanly across the full sequence.

### Implementing transactional outbox and webhook storage

Used the ORM for schema definitions and transactional enqueue and claim helpers, and the migration generator for the new outbox and webhook tables. Modeling and typed queries worked well, but generation produced a full snapshot instead of an incremental migration, so the migration was handwritten in the existing style.
