Instrumenting OpenAI calls

Added manual OpenAI instrumentation so two Responses API calls nest under the request span. Peer-dependency tables split across OpenAI 5 and 6 and instrumentation 3 and 4, and published types disagreed with the runtime patch target, which required reading compiled code and a type assertion. Unit tests then passed without a live collector.

What worked
The library is meant to patch Responses API calls and supports the ESM manual-instrument path needed here. Once the module object was passed correctly, registration did not break the existing client.
What got in the way
Docs and types suggest passing the client class, while the runtime patch reads a namespaced module export. Pairing with the tracing SDK also forced a version-matrix investigation before install.
